Cigarette smoking is an important risk factor in the etiology of atherosclerotic heart disease which is an important health problem for Turkey and world. The aim of this study was to determine the effects of angiotensin converting enzyme inhibitors, widely used group of drugs for different cardiovascular diseases, in healthy smokers. Thirty-eight healthy volunteers (29 man, 9 woman, mean age 36.4±5.5) smoking more than 20 cigarettes /day and 30 non-smoker volunteers (21 man, 9 woman, mean age 34.1 ± 6.1) were included in the study. For both groups HbA1c, cholesterol, triglycerides, HDL-cholesterol, LDL-cholesterol, insulin, urea, uric acid, ALT, AST, creatinin, Na, K, Cl, Mg, Ca, and urinary albumin/creatinine ratio were measured. While HDL-cholesterol was found to be significantly lower in the smoker group (46.7 ± 3.6 mg/dl vs 49.9 ± 3.4 mg/dl, p <0.05), urinary albumin/creatinine ratio was significantly higher when compared to non-smoker group (0.017 + 0.008 vs 0.011 + 0.007, p <0.05). There is no significant difference for other parameters between groups (p>0.05). Then smoker group was divided into two groups and one group was received 5 mg lisinopril daily (treatment group) and the other group was received placebo (placebo group). Uric acid (6.45 + 0.95 vs 5.35 + 1.22 mg/dl, p<0.05)and magnesium (2.05 + 0.33 vs 1.78 + 0.22 mg/dl, p?0.05) levels were significantly reduced in the treatment group, other parameters were not changed in both groups ( p>0.05). In conclusion HDL cholesterol was reduced and urinary albumin excretion was increased in healthy smokers, with short term lisinopril treatment these changes were not reversed, and serum uric acid and magnesium levels were reduced. Effects of ACE inhibitors on urinary albumin excretion and cardiovascular risk factors in smokers and long term consequences of this therapy in terms of CVS morbidity and mortality should be investigated further.

Bu çalışmanın amacı, son zamanlarda çeşitli kardiyovasküler hastalıklarda geniş kullanım alanı bulan "angiotensin converting enzyme" (ACE) inhibitörlerinin sigara içen sağlıklı insanlarda aterosklerotik risk faktörleri üzerindeki etkilerinin araştırılmasıdır. Çalışmaya günde en az 20 sigara içen 38 sağlıklı birey ( 29 erkek , 9 kadın; yaş ortalaması 36.4±5.5) ile (sigara grubu) sigara içmeyen 30 sağlıklı birey alındı (21 erkek, 9 kadın; yaş ortalaması 34.1±6.1). Bütün olgularda Hb A1c, glukoz , kolesterol, trigliserid, HDL-kolesterol, LDL-kolesterol, insülin, üre, ürik asit, ALT, AST, kreatinin, Na, Cl, K, Mg, Ca ve idrarda albumin/ kreatinin değerleri saptandı. Sigara içen grupta HDL-kolesterol seviyesi kontrol grubuna oranla düşük bulunurken (46.7 + 3.6 mg/dl’ye karşı 49.9 + 3.4 mg/dl, p <0.05), idrarda albumin/kreatinin oranı yüksek bulundu (0.017 + 0.008’e karşı 0.011 + 0.007, p <0.05). Diğer parametreler arasında istatistiksel açıdan önemli bir fark yoktu(p>0.05). Sigara içen grup daha sonra ikiye ayrılarak birinci gruba 10 gün süreyle 5 mg/gün lisinopril verilirken (tedavi grubu) diğer gruba plasebo uygulandı (plasebo grubu). Tedavi grubunda ürik asit (6.45 + 0.95’ e karşı 5.35 + 1.22 mg/dl, p<0.05)ve magnezyum (2.05 + 0.33’e karşı 1.78 + 0.22 mg/dl, p?0.05) seviyelerinde düşüş görülürken, diğer parametrelerde anlamlı bir değişiklik olmadı ( p>0.05). Sonuç olarak sigara içen sağlıklı bireylerde serum HDL-kolesterol düşerken üriner albumin atılımı artmıştır. Kısa süreli lisinopril uygulaması bu artışı geri döndürememekte, buna karşılık serum magnezyum ve ürik asit düzeylerini düşürmektedir.
