| 1. | Cephalosporins and Carbapenems Sensitivity of Staphylococci Isolated from Raw Milk Samples S. Aslıhan Cengiz, Güven Uraz Pages 43 - 46 Aim and Method: 16 of coagulase positive staphylococci (CPS) and 22 of coagulase negative staphylococci (CNS) were isolated from milk samples taken at several time periods in a milk factory. Resistance and susceptibility rates of them against both cephalosporins (cephradine: CE, cefuroxime: CXM and ceftizoxime: ZOX) and imipenem (a carbapenem) were coincidentally investigated. Results: Susceptibility rates of total 38 isolates for CXM, CE, ZOX and IPM were18.42%, 26.31%, 7.9% and 13.16%, respectively. Conclusion: This finding reflects the necessity for choice of therapy regimens according to the susceptibility tests in these CNS and CPS infections. |
| 2. | The Treatment of Gastrointestinal Foreign Bodies in Childhood Burhan Köseoğlu, Vedat Bakan, Salim Bilici, Önder Önem, İsmail Katı, İsmail Demirtaş Pages 47 - 53 Aim: Gastrointestinal foreign bodies continue to become a serious health problem for childhood. In this retrospective clinical study, we investigated the methods used for the removal of gastrointestinal foreign bodies, the structure and location of foreign bodies, and success rates of the treatment. Method: Between June 1995 and February 2000, 74 patients with gastrointestinal foreign bodies were treated in our department. Results: Sixty-five foreign bodies (85.5%) were metal objects. Most of the extracted foreign bodies were coins located in the proximal esophageal sphincter. Foreign bodies were extracted endoscopically in 61 (82.4%), were pushed into the stomach in two patients (2.7%), while spontaneously went through the outlet of the stomach in nine patients (12.1%). Surgical removal was performed on in two patients (2.7%). There was no major morbidity or mortality. Conclusion: Sharp and pointed foreign bodies in the stomach and duodenum should be removed using endoscopy. Blunt foreign bodies in the stomach and all foreign bodies in intestine should be managed conservatively When patient has peritoneal irritation signs or foreign body persisting İn the same location, surgical intervention should be planned. Endoscopic approach is still the most preferred method for the removal of pediatric esophageal foreign bodies, because its high success rate, and safety. |
| 3. | Our Experiences in Surgical Treatment of Esophageal Cancer: Analysis of 57 cases Çetin Kotan, Erol Kisli, Reşit Sönmez, Murat Aslan, Abbas Aras, Hasan Arslantürk, Ömer Söylemez Pages 54 - 60 Aim: Esophageal cancer is frequently seen gastrointestinal cancer in Van region. Method: 57 esophageal cancer cases were operated in our clinic between 1994-2000. Results: Tumors were localized at cervical in 3, intrathoracic in 21, and distal esophagus in 33 cases. Resectability rate was found as 91% and esophagectomy were performed in 52 cases with different surgical techniques. Laparotomy, right thoracotomy and cervical esophagogastrostomy was most frequently (43%) used surgical technique in intrathoracic tumors, and laparotomy, right thoracotomy, distal esophagectomy proximal gastrectomy and intrathoracic esophagogastrostomy (36%) in distal tumors. Anastomosis performed with sirculer stapler in 21 cases. Anastomotic leakage was not seen in stapler used cases, but leakage was occured in 4 cases (7.4%) which stapler is not used. Total complication rates was found as 56%, and hospital mortality was as 10.5% Conclusion: The different surgical techniques in therapy of esophageal cancer may be preferred according to localization of tumor and decision of surgeon. |
| 4. | To Compare of Trabeculectomy Results in Middle and Advanced Age Groups Tekin Yaşar, Murat Özdemir, Şaban Şimşek Pages 61 - 64 Aim: To evaluate results of the trabeculectomy operations and to compare success rate according to the ages in middle and advanced ages of primary open-angle glaucoma (POAG) patients. Methods: Trabeculectomy operations were performed to 52 eyes of 32 cases which intraocular pressures (IOP) were uncontrolled with maximal medical treatment between February 1997-June 1999. The patients were divided into two groups: under 60 years ages (Group 1) and over 60 years ages (Group 2). Twentyl two eyes were in group 1 and 30 eyes in group 2. Postoperatively, IOP under 21 mmHg without medication was accepted as success. Postoperative examinations were done at 1st and 2nd weeks, 1st, 2nd, 3th, and 6th months. Results: Mean IOP decrease was 14.59±4.97 mmHg in group1 and 13.56 ±3.11 mmHg in group 2 at the postoperative 6th month examination. Success rate was 91.32% in group 1 and 93.21% in group 2. There was no corelation between IOP decrease and ages with Pearson corelation coefficient test (rp=- 0.21, p= 0.13). Conclusion: We suggest that results and success rates of trabeculactomy is not changed in middle and advanced ages of uncomplicated POAG patients. |
| 5. | Cerebellopontine Angle Tumoursi MRI Findings Özkan Ünal, Ömer Etlik Pages 65 - 70 Aim: Tumours of the cerebellopontine angle are essentially benign in adults, and require a preoperative assessment as precise as possible by MRI. Preoperative diagnose and differentiation between acoustic neurinoma and meningioma of the cerebellopontine angle is important in selection of the surgical approach, successful tumor removal, and preservation of hearing and facial nerve. We aimed to present MRI findings of cerebellopontine angle tumours, retrospectively. Method: We reviewed MRI in 20 patients with 21 cerebellopontine angle tumours included 7 meningiomas, 9 acoustic neurinomas, 2 epidermoid tumors, 2 hemangioblastomas and 1 cerebellar astrositoma. Results: MRI examinations were reviewed with special attention to the size, signal intensities, patern of contrast enhancement and relations of the tumor to the adjacent tissues. Conclusion: Preoperative diagnosis and differentiation of cerebellopontine angle tumours. MRI is capable of providing excellent images of the cerebellopontine angle and contents of internal auditory canal rather than all other diagnostic modalities. It is concluded that MRI is the most valuable diagnostic methods of cerebellopontine angle tumours. |
| 6. | Isolated Iliac Artery Aneurysm Anıl Z. Apaydın, Melda Apaydın, Faik F. Okur, Ali Telli Pages 71 - 72 Isolated iliac artery aneurysms are rare and difficult to detect. They entail a high risk of rupture. We report a case of 76 year-old patient with a giant isolated iliac artery aneurysm who was successfully treated. Risk of rupture is related to the size of the aneurysm. Surgical mortality is associated with the presence of rupture, not with the size of the aneurysm. |
| 7. | Hydatid Cyst Which Was Situated in Muscle Near the Femoral Artery Mehmet Özkökeli, Mehmet Ünal, Bingür Sönmez Pages 73 - 75 Echinococcus alveolaris is an endemic infectious disease in our country. We reported a case of hydatid cyst which was situated within muscle near the femoral artery. Previously, the patient was carried on femoro-femoral bypass operation, there was a pulsatile mass in left femoral region in physical examination 5 months later. In ultrasonographic evaluation, a cystic lesion was seen. After femoral exploration, the patient was taken for surgery. The surgical approch was cystic resection. Echinococcus alveolaris was seen in histopathological examination. The patient was discharged at the 6 th postoperative day without any complications. |
| 8. | Brunner’s Gland Adenoma (A Case Report) Kısmet Bildirici, Bahattin Erdoğan Pages 76 - 78 Brunner’s gland adenoma is a rare benign tumor of the duodenum that is usually small and asymptomatic. Benign duodenal tumors are much less common, representing approximately 0.008% of all tumors found at surgery and autopsy. Tumor arising in the duodenal glands make up approximately 10.6% of benign duodenal tumors, and is thus extremely rare. The majority of Brunner’s gland adenoma occur in the proximal duodenum. Their diameter ranges in general from a few mm to approximately 2 cm. The lesions are usually detected by endoscopy and small tumors can be excised. In the present article, a case of Brunner’s gland adenoma associated with pancreatic carcinoma in a 53 year-old women is described. |
